What requires careful attention here is that this refers to the probability of harm occurring, not the probability of a defect occurring. At first glance, one might think that if there is a defect, it should simply be corrected. However, actual risk assessment is not that straightforward.

This definition has profound meaning. What matters most for medical devices is “patient safety.” Even if a product has some malfunction, if it does not actually cause harm to patients or users, it is evaluated as low risk.

For example, if a thermometer has a defect that causes its display to deviate by 0.1°C, but this is within a range that does not affect patient treatment, it does not lead to harm. On the other hand, even a slight malfunction in a pacemaker can lead directly to life-threatening harm.

In ISO 14971 and FDA (U.S. Food and Drug Administration) guidance, it is common to express the probability of occurrence of harm as the product of two elements:
